Driver Employment Application: What It Must Include

For motor carriers and hiring teams: This page explains the driver employment application required by 49 CFR § 391.21. It is not an application to work for LBC Fleet, and it is not a place to submit a completed driver form.

Start with the carrier’s current application

The motor carrier furnishes the form. The applicant completes and signs it. The required information includes identity and residence history, current commercial licenses or permits, driving experience, three years of crashes and moving-violation history, license-denial or suspension history, and employment history. The rule also requires the applicant’s date of birth and Social Security number, so a completed form needs a secure hiring channel—not a public contact or sales-quote form.

Employment history has two parts: the preceding three years of employers for every applicant, plus an additional seven years of commercial-motor-vehicle operating employers for applicants who will operate a vehicle covered by Part 383. See our driver employment-history guide for that distinction.

The application is the start, not the whole file

A carrier must also handle the applicable prior-employer and driver-record inquiries under 49 CFR § 391.23. Before submission, the carrier must give the notices about safety-performance-history checks and the applicant’s rights described in § 391.21(d). Review the current text of 49 CFR § 391.21 and the related § 391.23 investigation requirements when setting up your process.

Looking for a form?

Ask the hiring motor carrier for its current application and approved secure submission method. We are not offering an old PDF as a current fill-and-send form here. Employers who need help organizing driver qualification records can read about LBC Fleet’s driver qualification file management.
