Driver Employment History for DQ Files
Building a driver qualification file? The driver’s employment history belongs in a signed employment application. A short “Employment Record” worksheet may help collect details, but it does not replace a complete application or the carrier’s separate prior-employer inquiries.
How much history does the application need?
49 CFR § 391.21(b)(10) calls for the applicant’s employers during the preceding three years, including employment dates and reasons for leaving. Under § 391.21(b)(11), an applicant who will operate a commercial motor vehicle as defined by Part 383 must also list employers during the seven years before that three-year period for which the applicant operated a CMV. That is not the same as saying every driver must list ten years of every driving job.
The regulation also asks whether the applicant was subject to the FMCSRs at a previous employer and whether the prior job was a DOT-regulated safety-sensitive function subject to alcohol and controlled-substances testing. The carrier’s own application must contain the other required fields and the applicant’s signed certification; this page is not a substitute for it.
Application history is not the whole check
49 CFR § 391.23 separately requires specified motor-vehicle-record and prior-employer safety-performance inquiries. The carrier should document its requests, responses, and good-faith follow-up in the appropriate driver records. Do not treat a completed applicant worksheet as proof those inquiries were done.
The older LBC version 2.3 Employment Record PDF uses a broad “10 years for driving jobs” instruction and is not linked here as a current stand-alone form. If you need help collecting and organizing applications and authorized prior-employer follow-up within a defined service scope, see LBC’s driver qualification file management service. The motor carrier remains responsible for qualification decisions and regulatory compliance.
