DOT Test Consent: Know Which Document You Need
For motor carriers and CDL drivers: “Test consent” can refer to several different steps. This page explains the distinction; it is not a testing order, a Clearinghouse authorization, or a form to send to LBC Fleet.
Which test is being ordered?
For drivers covered by FMCSA’s Part 382 program, an employer generally must receive a verified negative controlled-substances result before the driver first performs safety-sensitive work, unless the specific exception in 49 CFR § 382.301 applies. Pre-employment alcohol testing is optional under that section, with conditions if the employer chooses it. Other test reasons—such as random, post-accident, reasonable suspicion, return-to-duty, and follow-up—have their own rules. The employer should identify the actual reason and use the current DOT testing process rather than a preselected, generic release.
Three records that should not be confused
- Testing policy receipt: 49 CFR § 382.601 requires the employer to give covered drivers its drug-and-alcohol educational materials and retain a signed certificate that the driver received them. A receipt is not a test result.
- Clearinghouse query consent: FMCSA explains that a limited query uses general consent obtained outside the Clearinghouse, while each full query requires specific electronic consent inside it. A general “test consent” sheet does not replace those steps.
- Test documentation: The carrier and its testing partners use the applicable DOT process and records for the actual test. Keep driver identifiers and results in the approved controlled system, not a public contact form or ordinary sales inquiry.
Why there is no old PDF here
An older LBC release was prepared for a previous workflow and displays one test reason as selected. We are not presenting it as a current, all-purpose form. Ask your employer or testing-program administrator for the current instructions and the correct document for the specific test. For employers seeking broader administrative support, see LBC Fleet’s compliance and driver-support service directory; it does not replace the carrier’s testing responsibilities.
